中醫辨證論治

有些疾病無症可辨,有些疾病併發症太多,很難歸經、辨虛實,偏頭痛正是如此,病源障礙發生在腦幹、大腦皮質、枕葉神經功能失調引起許多症狀,因此需深入研究。

通竅活血湯

屬瘀血阻滯頭面者:血瘀腦絡、頭痛眩暈、耳聾、頭痛經久不愈、痛處固定頭痛,胸脅疼痛,內熱煩悶,急躁善怒,失眠多夢,心悸、呃逆乾嘔;屬瘀阻頭眼者:瘀血停滯、視網膜中央動脈栓塞,眼痛如炸、暴盲、舌赤瘀斑,脈弦滑等。

補陽還五湯

屬氣虛血瘀腦部病變:頭痛頭暈、綿延不已、失眠多夢、心悸怔忡、勞則更甚,倦怠乏力、食慾不振面色恍白舌質偏淡,脈細弱。

藥方組成:

吳茱萸湯(傷寒論):吳茱萸、人參、生薑、大棗。

半夏天麻白朮湯(脾胃論):半夏、天麻、白朮、黃柏、澤瀉、陳皮、神麴、生薑、黃

耆、乾薑、麥芽、人蔘、茯苓。  

天麻鉤藤飲(雜病證治新義):天麻、夜交藤、鈎藤、石決明、梔子、川牛膝、黃芩、益母草、杜仲、茯神、桑寄生。

血府逐瘀湯(醫林改錯):红花、當歸、生地黃、牛膝、赤芍、枳殼、川芎、桔梗、柴胡、甘草。

通竅活血湯(醫林改錯):赤芍、生薑、川芎、桃仁、紅花、大棗、老蔥。

補陽還五湯(醫林改錯):黃耆、歸尾、赤芍、地龍、川芎、桃仁、紅花。

偏頭痛常用單味藥物:

血藥、風藥、腸胃藥三合一隨症酌加如下:

1、血藥:川芎、川七、地龍、水蛭、赤芍、紅花、桃仁、當歸、乳香、沒藥、人蔘。

2、風藥:天麻、鈎藤、全蠍、金蜈蚣、羌活、防風、蔓荊子、白芷、生薑、麻黃、藁本、葛根、薄荷(巔頂惟有風藥可及)。

3、腸胃藥:元胡索、半夏、吳茱萸、丁香、神麴、生姜、竹茹、柿蒂、陳皮、麥芽、枳殼。

※以上藥材皆採用科學濃縮中藥;除全蠍、金蜈蚣之外。

偏頭痛針灸常用 1、2 風池、風府、太陽、大椎、百會、肩井、阿是、天柱、列缺、太衝、太谿、足三里、內關、中脘、頭維、陽白、印堂、攢竹

頭痛部位循經配穴 12

前額部(足陽明經)取足陽明經穴為主:陽白、攢竹。

偏頭痛(足少陽經)取足少陽經穴為主:風池、太陽、頭維。

後頭痛(足太陽經)取足太陽經穴為主:風池、風府。

頭頂痛(足厥陰經)取足厥陰經穴為主:太衝、百會。

針灸治療偏頭痛,可以刺激神經、改變大腦與脊椎神經的訊息,且針灸被認為可以促進腦內啡的分泌增加與其他神經傳送素的釋放,腦內啡是身體分泌的天然鎮痛物質。

推拿、按摩是最古老的醫術,用以治療偏頭痛亦有其特殊的療效,就以無痛針灸而言,在針灸之前先按摩穴道,疎解經絡、神經之後再進針,如此可減輕進針時所造成的局部疼痛,由此可知推拿、按摩對於神經、肌肉、關節、骨骼、血管……等功能的恢復有相當肯定的效果。

推拿、按摩穴位 12 如:肩井、風池、風府、太陽、百會、大椎、阿是、天柱、頭維、陽白、印堂、攢竹

偏頭痛個案體質因素差異懸殊,虛者-腦失所養,實者-經脈瘀滯,若要以標準化治療是不可能的,每個病人都必須個體化,詳細辨證論治,並且急者治其標,緩者治其本,成功率將大幅提升;偏頭痛屬於慢性病,但病患常因急性發作期,才急迫尋醫,一方面受到西藥影響,止痛藥、止吐藥、鎮定劑起效迅速(病患很快滿意),但又擔心西藥引起抗藥性、藥隱、副作用……等,所以,等病痛復發時才急迫使用止痛藥,平時不得服用,又誤將中藥當成上述;中醫若無法迅速減輕症狀,病患常會另尋他人醫治(因痛苦難奈),因此「症狀療法與病因療法」缺一不可,必須標、本同治才會成功。

 

 

 

誘發因素

1、食物方面如乾酪、熱狗、味精、喝酒、吸煙、巧克力、起司、橘子、茶葉,柳橙、蕃茄……等,常會引發偏頭痛發作,但並非每人相同,得視個人體質敏感度而定

2、月經方面:偏頭痛的女性患者常於經期前1~3天復發或加重,另外,懷孕中後期會減輕或甚至不發作。

3、藥物方面:某些口服避孕藥、血管擴張劑、神經興奮劑……等,也會引起偏頭痛發作。

4、其它方面:如經神壓力、精神緊張、情緒波動、焦慮、疲勞、熬夜、飢餓、季節更換、天氣變化過大、感冒、血糖過低、頭部外傷……等,以上各種方面因素,都可以激起偏頭痛的發作。

  The pain is located above the eyebrows, either one-sided or two-sided, or spreading over the forehead. TCM claims that the pain is mainly caused by invasion of wind-heat pathogens or wind pathogens along with stasis. Also, dysfunctions of the <a href="../principles/stomachmeridian.html" class="link">stomach</a> and <a href="../principles/gallbladdermeridian.html" class="link">gallbladder meridians</a> have played important roles in the development process.</p>
